Consent to Direct Marketing

Pursuant to Article 10(2) of the Act on the provision of electronic services, I consent to receiving commercialinformation in the form of a newsletter to the e-mail address I have provided.

I consent to the processing of my personal data by the Personal Data Administrator – Fundacja K.I.D.S. Klub Innowatorów Dziecięcych Szpitali with its registered office in Warsaw – for the purpose of direct marketing bysending a newsletter by the Administrator to my e-mail address.

The data will be processed for this purpose pursuant to Article 6(1)(a) of Regulation (EU) 2016/679 of the EuropeanParliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing ofpersonal data and on the free movement of such data, and repealing Directive 95/46/EC (GDPR).

The data will be stored until I withdraw my consent.

I have been informed by the Administrator that I have the right to access my personal data and the right to rectify,erase, restrict processing, the right to data portability, the right to object, and the right to withdraw consent atany time without affecting the lawfulness of processing carried out on the basis of consent before its withdrawal.
