Consent to the Processing of Personal Data

I hereby consent to the processing of my personal data in the following scope: first and last name; street; house number;apartment number; postal code; city/town; country; contact phone number; e-mail; company name; Tax Identification Number (NIP),for the purposes of providing services to me by the Administrator – Fundacja K.I.D.S. Klub Innowatorów Dziecięcych Szpitali,ul. Chmielna 73, 00-801 Warsaw, KRS: 0000793389, REGON: 383824678.

I declare that, pursuant to Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on theprotection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealingDirective 95/46/EC (General Data Protection Regulation), I have been informed that:

• the controller of my personal data is the Fundacja K.I.D.S. Klub Innowatorów Dziecięcych Szpitali, ul. Chmielna 73,00-801 Warsaw, KRS: 0000793389, REGON: 383824678;
• my personal data will be processed solely for the purposes indicated above and will be disclosed to third parties, where therecipients of the data will be entities providing services to the controller – on the basis of personal data processingagreements (data processing entrustment agreements);
• my personal data is processed in order to perform the contract between me and the controller and, on the basis of my consent,for direct marketing purposes;
• my personal data will be stored for the duration of the contract concluded between me and the controller;
• I have the right to request from the controller access to my personal data, rectification, erasure or restriction of processing;I have the right to object to processing and the right to data portability;
• I have the right to withdraw my consent to the processing of my personal data at any time, without affecting the lawfulness ofprocessing carried out on the basis of my consent before its withdrawal;
• I have the right to lodge a complaint with a supervisory authority;
• the provision of personal data is voluntary, and failure to provide it will result in the inability to perform the contractbetween me and the controller;
• the controller collects data in an automated manner but does not carry out profiling with regard to me;
• I declare that I am of legal age and have full legal capacity. I also declare that I have read the content of the above consentin full and understand it completely.
